Clinical eroticism
Clinical eroticism is a term from the field of sadomasochism. It involves sexual practices related to situations involving medical treatment, hospital stays, or medical examination methods, usually incorporating them into an erotic role play between sexual partners (doctor - patient, doctor - nurse - patient).
The term is often used for doctor play, hospital sex, or white eroticism. The term is derived from the white coat, as medical personnel predominantly wear white clothing.

An enema is the injection of a fluid into the rectum. During the Renaissance, an enema was a common medical practice. Prescribed as a panacea by the personal physician, it could often lead to stimulation through touching the rectal area, thus leading to sexual addiction. This is called clysmaphilia or enema mania and is a form of fetishism.
There are different types of enemas: enema, cleansing enema, irrigation, high enema, siphoning or swivel enema, rectal colonic irrigation, etc.

Single-use enema: Single-use saline enemas are used by nursing staff in hospitals or doctor's offices and are also called mini enemas. Single-use enemas consist of a plastic container containing the enema fluid, fitted with a rectal cannula. Single-use enemas are intended for single use only and are disposed of after use. After the solution has been administered into the rectum, the patient must make an effort to retain the highly concentrated solution for a while. Single-use enemas should only be used sporadically and for short periods of time, as the enema fluid can cause disturbances in the body's water and mineral balance. Microclysters: Microclysters are often used for children. Such a microclyster contains a minimal amount of highly concentrated, viscous salt and is also available ready-to-use with an applicator. There are also special ready-to-use microclysters with anticonvulsants for interrupting acute epileptic seizures. These usually contain small amounts of the respective medication and are also used in adults. Microclysters should only be used on medical advice, especially in children.
Special fetishes
Saliromania or desecration drive refers to a sexual practice in which the soiling of people or objects is associated with sexual arousal. However, it is not an urge or a mania, as the terms might suggest.
The forms of saliromania are diverse – it always involves smearing or splashing oneself or one's partner with all kinds of slimy or liquid substances (paste, paint, mud, spit, semen, urine, or feces, etc.). In colloquial language, the preference for incorporating urine into sexual play is often referred to as urophilia. Because it sounds less clinical, the terms "peeing," water sports, pissing, lukewarm games, golden shower, golden waterfalls, and wet games are common in the corresponding scene. It is a harmless and varied sexual activity. Games with peeing are now considered normal in sexuality and bring great pleasure if both parties consent.
Like all sexual practices, urophilia can also be practiced within the context of sadomasochism. In sadomasochism, urine is often used to punish the submissive partner; in many cases, the SM practitioner derives satisfaction from humiliation. A precise distinction between reward in the sense of urophilia and humiliation in the sense of sadomasochism is difficult, as the two practices often overlap. Another variation in sadomasochism is urinary retention in the passive submissive partner, or control over excretion, such as inserting a catheter, forced urination, or closing the urethra and subsequent filling of the bladder.
Coprophilia, or caviar addiction, refers to the sexual pleasure derived from human feces or its excretion.
Arousal arises, for example, from observing bowel movements, the sight of feces, or even from direct physical contact. Holding back bowel movements to the point of loss of control and the resulting feelings can also lead to pleasure. Such fecal play can be purely autoerotic or practiced with a homosexual or heterosexual partner. One's own or another's feces is often smeared on one's own or another's body.
For some people, this tendency can extend to coprophagia, the eating of feces. Due to the concentration of bacteria and fungi in stool, this sexual practice poses a health risk. People with weakened immune systems should refrain from mucous membrane contact with feces.
Coprophilia is considered an abnormal sexual behavior. Although there is some connection to urine play, a tendency toward urophilia is not inevitable, and urophiles are generally not interested in feces play.
Common terms or codes for coprophilia are caviar, scat, chocolate, and nougat. Many film productions from South America or Asia – especially Japan – in which young girls demonstrate their needs in various scenes are popular. In commercial porn productions of this type, chocolate is sometimes (but not always!) used to create the impression of excrement.
Sexual practices
Sexual practices between two (heterosexual or homosexual) people include erotic massages, stimulation of erogenous zones such as the lips, tongue, earlobes, anus, and the entire body, petting—the stimulation of primary and secondary sexual organs, more intense stimuli such as beatings in sadomasochism, erotic electrostimulation, and various types of sexual intercourse. Depending on the usage, sexual intercourse refers to either sexual practices involving one or more primary sexual organs, penetration through the penis (vaginal, anal, oral), or just vaginal intercourse.
In addition, there are those practices that are not necessarily sexual but are perceived as sexually stimulating by those involved, such as role-playing, disguises or cross-dressing, intentionally delaying or accelerating sexual acts, sex in unusual locations, watching a pornographic film together, etc.
There are various possibilities for sexual contact or stimulation - these are described below.
During vaginal intercourse, the man's penis is inserted into the woman's vagina. It can be practiced in various positions (missionary, doggy style, riding, and many more – the Kama Sutra describes many exciting positions). Pregnancy can occur during vaginal intercourse. To prevent this, contraceptives such as condoms, diaphragms, or birth control pills are used.
This involves stimulating the partner's genitals with the hand or hands. The colloquial term 'handjob' has become common for the stimulation of the penis with the hand until orgasm. There is no popular term for the manual stimulation of the female genitals, and the term 'fingering' is occasionally used.
The stimulation of the partner's genitals with the hand is also referred to as masturbation, although this term is generally understood only as a technique for self-pleasure.
The technique varies from gentle stroking, tapping, rubbing, and rolling to more vigorous massage movements. Preferences for each type of touch vary greatly. As a rule, women tend to find lighter touches arousing, while men tend to find more vigorous touches, although this can also be the other way around.
Oral sex (also called 'French') refers to sexual intercourse with the mouth and tongue. The combination of mouth and penis is called fellatio (also called blow job), while the combination of mouth and clitoris or mouth and vagina is called cunnilingus (also called licking). During fellatio, the semen ejaculated during orgasm can be taken into the partner's mouth and possibly swallowed. Simultaneous mutual oral stimulation is colloquially called sixty-nine. The number '69' symbolizes the position of the heads, which lie between the legs on the partner's genitals and work on these pleasure zones with the tongue.
A more advanced form of oral stimulation is the insertion of the fully erect penis through the mouth into the relaxed throat of the partner with the neck stretched (deep throat) – however, this is only possible with mastery of the swallowing reflex. On the one hand, the penis is stimulated by the uniquely shaped laryngeal muscles, and on the other hand, it reaches some highly erogenous nerve strands in the throat.
Ass-to-mouth is a popular practice in pornographic films. Anal intercourse occurs first, followed by the passive partner taking the active partner's penis into their mouth. Due to the bacteria always present in the intestines, this sexual practice is associated with risks, which can be reduced by preparing the passive partner with an enema.
The term anal intercourse (also called 'Greek') refers to sexual intercourse in which the penis is inserted into the partner's rectum, i.e., through the anus (sphincter, also called rosette). In this variant, the passive partner must be particularly relaxed, otherwise pain or even injury can occur. With increasing sexual arousal, improved relaxation of the circular muscles around the anus is possible. For these reasons, appropriate patience and caution on the part of the active (penetrating) partner is required. Anal intercourse or anal coitus, colloquially also called ass fucking, refers to the insertion of the erect male penis into the sexual partner's anus. Anal sex is possible between a man and a woman as well as between a man and a man and, like vaginal intercourse, can be practiced in various positions (missionary position, a tergo, riding, etc. as with vaginal intercourse).
The insertion of a sex toy (dildo, butt plug, vibrator) or other objects, the fingers or hand (fisting), and the tongue (anilingus) counts as anal stimulation and not anal intercourse.
The anus is a sexually stimulable erogenous zone in both sexes, equipped with numerous nerve endings. In addition, passive anal sex in men leads to stimulation of the sexually sensitive prostate, which can trigger an orgasm. In women, anal sex can also stimulate the so-called A-spot and vaginal tissue.
In anal sex, the person inserting their penis is referred to as 'active,' while the person receiving the penis is referred to as 'passive.' Both can be experienced as pleasurable.
Neither the intestine nor the Cowper's glands produce enough natural lubricants, so a lubricant is generally required for anal sex. If proceeded carefully, the passive partner is able to relax the anus enough to allow penetration without pain, and anal sex can be experienced as pleasurable. Some find the pinpoint pain during penetration or when going too far particularly pleasurable. Overstretching the anal sphincter carries the risk of tears and fissures. Likewise, penetration too deep can lead to internal injuries in the rectum.
The passive partner can prepare for anal intercourse with an anal douching, such as an enema. Some anal sex enthusiasts also perform special anal stretching exercises beforehand.
For stimulation, the active partner can penetrate the passive partner with their fingers or objects such as dildos or butt plugs. A harness or strap-on dildo can also be used to use the dildo like an artificial penis – also known as 'strap-on'. This is often a form of sadomasochism and is called pegging. Anal stimulation can also be performed orally by the partner, by caressing the highly sensitive perineum, the external anal sphincter, with their mouth and tongue, also called rimming or anilingus, and even leading to penetration. Anal stretching is a term for exercise techniques aimed at stretching the anal sphincter.
Anal stretching can prepare the passive partner for anal intercourse so that the penis can be inserted pleasurably and painlessly. Anal stretching is also performed to enable pain-free anal stimulation during masturbation.
Anal stretching is practiced by both men and women. Rinsing with warm water in a shower or bathtub is perceived as pleasant, as it relaxes the anus.
To stretch the anal sphincter muscles, suitable objects are inserted rectally at regular intervals. A lubricant is used in the anus, and the objects gradually increase in size. Often used objects include dildos, vibrators, special butt plugs but also everyday objects such as baseball bats or zucchini, and the hands. The latter should ideally be worn with rubber gloves for safe fisting.
Nipples, earlobes, and the ear canals of many people are highly erogenous and so accessible to manual or oral stimulation that they can be stimulated to orgasm. The nipples, in particular, change noticeably, changing color, contracting, and hardening or becoming erect.
Tribadism refers to the rubbing of the labia and the clitoris between women (vulva-to-vulva). Frotteurism (penis-to-penis) refers to the rubbing of the penis and/or testicles, scrotum, with a penis.
Fisting, also called fist fucking, is the insertion of individual fingers, the hand, or the forearm into a partner. It is practiced both homosexually and heterosexually, and depending on the individual, the vagina or anus is penetrated, reaching the highly sensitive vaginal and sphincter muscles and the nerve-rich erogenous rectum. This is a rather complex and time-consuming technique that requires certain aids such as lubricants (e.g., silicone or grease) and trimmed fingernails, as well as some familiarization on both sides.
Usually, only a few fingers can be inserted during the first few fisting attempts. Only after several attempts is it possible to insert several fingers or even the entire hand during vaginal fisting. Many women experience vaginal fisting as an extremely intense sexual experience, which usually also leads to an intense orgasm. Since the muscular vaginal tube, as the natural birth canal, is naturally highly stretchable, this type of play can be practiced regardless of age or pelvic structure.
This refers to the much rarer penetration of the vagina or the anus with both hands.
Inserting one hand each into the vagina and anus: Inserting both hands into the vagina, with the hands placed palm to palm or palm to the back of the hand. In the latter variant, the Gräfenberg zone is particularly strongly stimulated by the finger joints of the upper hand.
Gaping or dilatation, particularly popular in sadomasochistic circles, involves inserting the hands back to back into the vagina or anus and then pulling them apart with appropriate force to allow the penetrator or third party to view the rectum of the penetrated woman or the cervix.
The full or partial insertion of the hand or fist into the vagina is a masturbation practice in which the presumed Gräfenberg zone can be specifically stimulated. Inserting the hand from the front requires some skill, which is why beginners usually assume a kneeling position, the doggy style position, and the hand, or usually just the four fingers, is inserted vaginally from behind.
The anus is anatomically much narrower and less stretchy than a vagina. Nevertheless, after initial fisting attempts, in which only a few fingers can be inserted, it is possible to insert several fingers or the entire hand. While several fingers are usually not a problem, the real difficulty lies in inserting the thumb and eventually the entire hand. This can be achieved – after some practice – with slight twisting movements. We do not recommend this practice due to the risk of injury!
Women who cannot orgasm during vaginal penetration with the penis can often become so aroused by cunnilingus that they experience orgasm The highly sensitive areas of the vulva, such as the particularly erogenous clitoris, can be better stimulated through oral contact.
Cunnilingus offers a wide range of stimulation possibilities: sucking, gentle to firm biting, breathing, a variety of gentler and possibly more forceful tongue movements such as licking, tapping, vibrating, stroking, etc. Even vocal sounds such as a quiet hum can have a stimulating effect due to the gentle vibrations they create.
Many women find it pleasant when their partner simultaneously stimulates the vagina or the anus with their fingers, for example through penetration, pressure, vibrating, or massaging movements.
Often, a couple only discovers over time the best method for cunnilingus that is particularly pleasant and arousing for each partner, and the intensity increases with more frequent and intensive use.
Fellatio is the term used for oral sex – the penis is stimulated by the sexual partner's mouth, tongue, lips, or teeth, and in the case of so-called deepthroating, also by the throat region. Sucking and sucking on the penis are particularly common.
Fellatio is an ancient sexual practice. Fellatio is often practiced during foreplay, both to increase sexual desire and, if necessary, to induce an erection.
